Pre-Participation Forms

All forms are due by August 1st.

Please go through each link below for directions to access all forms.

In additions to the forms for Sports Medicine, all student-athletes must have completed health forms on file at Hood Health Services prior to being cleared for participation.

Please note: physical must have been completed within 6 months prior to the start of athletic participation.

Please DO NOT submit forms by email. Form must be uploaded on Sportsware, faxed to 301-696-3488, or mailed to Sports Medicine, 401 Rosemont Ave. Frederick, MD 21701.

Verification of sickle cell trait status required. A copy of newborn screening record or copy of blood test should be submitted with physical.

REMINDER: While the college accepts physical completed in the previous year, athletics required physicals to have been done in the 6 months prior to starting sport participation. If you submit a physical to Health Services that has been done more than 6 months prior to participation, you will need to submit a new physical to Sports Medicine
